Several people (Kyle, for one - thanks man!) have informed me that this is a DUT (device under test) board, made by Advantest America. More info here.
It measures about 1/4" thick, and about 3.5" in diameter.
It's pretty cool looking non-the-less, and makes a great coffee mug coaster ; )
